SCHEDULE A CONSULTATION

• BEFORE YOU MAKE AN APPOINTMENT FOR SERVICE:
• Read CONTRAINDICATIONS to make sure you are a candidate.
• Have a Consultation with me if you are a first-time client. You may call me to book a consultation, or book one directly HERE.

PLEASE READ:

•   A 30% deposit is required to reserve an appointment for service. Please note that when you make an appointment, you are agreeing to the Terms of Service and Cancellation Policy of Plasma Precision by Kelly. 
• I accept credit cards, Zelle, cash, and WithCherry Payment Plan. The balance owing for services is due at your appointment prior to services commencing.
• 48 hours or more prior to your appointment, your deposit is refundable, with the exception of the $100 consultation fee and any credit card fees incurred from the credit card transaction that reserved your appointment.
• As a courtesy, I will send a confirmation text 24-48 hours prior to the scheduled service. Should this appointment reminder fail to reach you for any reason and you do not receive an appointment reminder, it is still your responsibility to manage your appointment and adhere to the cancellation policy.
• If you cancel less than 24 hours before your appointment or are more than 30 minutes late to your appointment, you forfeit your $100 consultation fee credit (or incur a $100 cancellation fee), your 30% deposit becomes non-refundable, but your 30% deposit may be applied to a rescheduled appointment. (The below rescheduling terms will apply.)
• You may only reschedule 2 times without forfeiting your 30% deposit. 
• Your treatment must take place within 60 days from your cancelled (original) appointment date, or you forfeit your 30% deposit.
• Your 30% deposit is forfeited if there is a "no show" at the time of your appointment.
• Refunds on remaining unused treatments from a purchased package will be given only after applying the full standard price of used treatments. Refund will be given in the form of a credit that may be used towards a different treatment or merchandise.

Due to these lengthy appointments, I expect that you keep your scheduled time with me, although I understand that emergencies happen. I strictly adhere to these policies for the benefit of all my clients, and I appreciate your understanding. Please do not bring children, pets, or other guests with you, with the exception of your needing assistance due to a contraindication. 

Please reschedule if you are ill or have been exposed to COVID.

Please reschedule if you have: fever or chills, cough, sore throat, congestion or runny nose, shortness of breath or difficulty breathing, fatigue, muscle or body aches, nausea, vomiting, diarrhea, headache, new loss of taste or smell.
